Chain of Custody Form: Please submit your
samples on a MVTL, Inc. Chain of Custody Form, which can be
used for water, wastewater, biosolids, soils, etc. Sample identification, date and time
of sampling, and requested analyses must be included on the form. There are areas for
noting report addresses, billing address, contact, phone and fax number. Fill in all
applicable information.
